세계보건기구가 2030년까지 바이러스 간염 퇴치를 달성 목표로 제시하였고, 해당 감염병 다발생 지역에서는 발생률 감소방안 마련을 위한 정책 연구 및 사업을 진행 또는 준비 중에 있으나, 의사결정을 위한 지역단위의 객관적 근거는 매우 제한적이다. 따라서 호남권 질병대응센터에서는 C형간염 다발생 지역인 호남권에 대한 C형간염 발생률 및 변화 추세를 시도, 시군구 단위로 세분화하고 이를 정량화한 지역단위의 통계지표를 제공하고자 한다. C형간염의 전수감시가 시작된 2017년 6월 3일부터 2021년 12월 31일까지 호남권에서 질병관리청에 신고한 자료를 바탕으로 분석한 결과, 호남권의 C형간염 조발생률과 연령표준화 발생률은 2018년에 가장 높았고 가장 최근인 2021년에 가장 낮은 것으로 확인되었으며, 시도 간 최대 1.9배, 시군구 간 최대 74.9배의 지역 간 발생률 차이가 관찰되었다. 호남권 C형간염 연령표준화 발생률에 대한 조인포인트 회귀분석 결과, 전수감시 기간(4.58 인년) 동안 통계적으로 유의한 감소 추세가 확인되었다(-12.0%/년, p=0.026). 또한 호남권에서 발생한 C형간염은 시도, 시군구 단위 및 성별에 따라 발생률과 변화율의 차이가 있는 것으로 나타났고, 전체적인 감소추세와 반대로 C형간염 발생 증가 경향을 보인 시군구도 관찰되었다. 본 보고서는 호남권에서의 C형간염 발생률 관련 통계지표로 조발생률, 연령표준화 발생률, 그리고 연령표준화 발생률에 대한 연간 % 변화율(annual percentage change, 이하 APC) 및 평균 연간 % 변화율(average annual percentage change, 이하 AAPC)을 제시하였다. 지자체는 정책 방향이나 목적에 따라 필요한 통계지표를 선택 조합함으로써 향후 호남권역 C형간염의 예방 및 관리에 필요한 의사결정에 객관적 근거로 활용하기를 기대해 본다.

Abstract
The World Health Organization has proposed the goal of eliminating viral hepatitis infection by 2030. Policy research and projects to reduce the incidence rate are ongoing in local governments where hepatitis C infection is common. However, a clear basis for decision-making is required at the regional level. Therefore, Honam Regional Center for Disease Control and Prevention intends to determine the incidence rate and trend of hepatitis C virus (HCV) infection by municipal levels in the Honam region, where HCV infection is prevalent. The analysis of data reported to the Korea Center for Disease Control and Prevention Agency in the Honam region from June 3, 2017, to December 31, 2021, revealed that the incidence rate of HCV infection in the Honam region was the highest in 2018 and the lowest in 2021. It varied by up to 1.9 times in cities and provinces and 74.9 times in municipalities. The result of join point regression analysis revealed a statistically significant decreasing trend during the mandatory surveillance period. In addition, there were differences in the incidence rates of HCV infection in the Honam region according to the cities, provinces, municipalities units and gender. The statistical indices can be selected and combined according to the policy direction or objectives of the local government. Hopefully, these findings will serve as an objective basis for making decisions necessary to prevent and manage HCV infection in the Honam region.
